We are an in-network provider for BCBS PPO & Choice plans, Aetna, Optum-UBH / United Healthcare, Cigna, and self-pay. HSA, HRA, credit cards, checks and cash are all acceptable forms of payment.

A 55-60 minute Initial Individual, Couples and Family Therapy intake session will be billed at the rate of $170
Subsequent 50-55 minute Individual, Couples and Family Therapy sessions are billed at the rate of $160
Subsequent 50-55 minute Group Therapy sessions are billed at the rate of $50
Sage Therapy Chicago considered out-of-network with insurance providers not listed above. Out-of-network means that we do not have a contract with your health insurance plan provider. However, it is still possible to use these benefits.
At the time of service, you pay the fee directly and we will provide you with a monthly “Superbill” to submit to your insurance provider for reimbursement. The reimbursement is paid directly to you. For more information and greater detail regarding your benefits, please call your insurance provider about coverage for our services.
As is common practice, clients can cancel or reschedule an appointment anytime, as long as they provide 24 hours notice. Failure to cancel an appointment, reschedule or show up with less than 24 hours notice, will unfortunately result in a charge for the full cost of the session, not your co-pay or co-insurance. In the event of an emergency or unforeseen events, the fee will of course be waived.
